Housing Consultation Services
Program Overview
Seniors and individuals with disabilities who don’t have a targeted case management, or currently not working with an MSHO/MSC+ Care Coordinator, and need housing stabilization services are eligible for this program. Our dedicated Housing Consultants will work with you to develop a Housing Focused Person-Centered plan, identify and establish eligibility, and assist with attaining housing stabilization services.
Eligible Services
* Developing a Housing Focused Person-Centered Plan based on assessment results.
* Assisting with accessing documentation required for Housing Stabilization Services eligibility.
* Supporting with identifying strengths, needs and wants in housing including cultural requirements and/or preferences.
* Supporting with making an informed choice in choosing a housing transition or sustaining provider.
* Coordinating with other service providers to ensure success of client's housing goals.
* Helping with understanding rights to privacy and appeal information.
* Annual update to the person-centered plan as needed to maintain housing stabilization needs and goals.
