Housing Sustaining Services
Program Overview
This program provides assistant and support to individuals to maintain and sustain living in their home in the community.
Eligible Services
* Developing, updating and modifying the individual's person-focused housing plan as needed.
* Early identification, coaching and preventing of behaviors that may jeopardize housing stabilization.
* Providing tenant education, coaching and trainings regarding occupancy roles, rights, and responsibilities.
* Providing transportation accommodation relating to housing services and related obligations.
* Promoting cultural understandings with landlords, property managers and community neighbors.
* Assisting with annual housing recertification application and process.
* Assisting with identifying and applying for financial benefits and other housing stabilization assistance to retain housing.
* Collaborating with property managers, landlords and housing agencies to promote housing retention.
* Arranging for assistive technology.
* Arranging for adaptive house related accommodations.
