Housing Transition Services
Program Overview
This program assist qualified individuals plan, search, and transition into a home.
Eligible Services
* Assist with developing a person-centered housing transition plan.
* Assisting in identifying housing benefits, eligibility requirements, and completing the eligibility application process.
* Assisting with the home search, completing applicable and necessary paperwork (i.e. rental/tenant applications), and supporting with the housing application process.
* Assisting with tenant screening and housing assessments.
* Providing transportation accommodation for housing services and other housing related obligations.
* Assisting with understanding the terms of the lease.
* Assisting with lease contract negotiation.
* Assisting with planning, organizing, and coordinating moving logistics.
* Performing move-in safety inspections.
* Arranging for necessary ADA accommodations as needed by the individual.
* Assisting with identifying and applying for financial assistance for security deposits, moving expenses, and home goods.
